On 9 September, Diaverum Estoril welcomed the Executive Committee of the Local Council for Social Action (CLAS) of Cascais for a meeting focused on strengthening coordination between healthcare services and social support available in the municipality.
Held in the year marking the 25th anniversary of the Cascais CLAS, the meeting, under the theme “Caring Through Collaboration”, provided an opportunity to present the Diaverum Estoril care model, gain a better understanding of how the Social Network operates and identify new opportunities for collaboration.
The advanced age and clinical complexity of people receiving haemodialysis are often associated with isolation, functional dependence, caregiver burden, transport difficulties and socioeconomic vulnerability. Responding to chronic kidney disease therefore requires an integrated approach encompassing clinical, social, family and community dimensions.
“Joining CLAS represents an opportunity to improve coordination between healthcare and social support, placing the person at the centre and promoting their quality of life, autonomy and inclusion,” says Jesús Garrido, Clinical Director at Diaverum Estoril.
The areas of cooperation identified include health literacy, healthy ageing, caregiver support, tackling social isolation, access to additional Social Network services and coordinated responses to situations of greater vulnerability.
The meeting also included a visit to the unit’s facilities and concluded with a commitment to further explore the opportunities identified and assess potential joint initiatives.
